Apart from the side marker holes, what are the main differences between a US and ROW front fender?

Will I encounter any fitting issues using a US fender on a Euro car, with euro headlights?

I've fitted Euro H4 headlights on both of my cars, so I think you will be ok with that. I can't speak for the rest. Hopefully someone more knowledgeable will speak.

I think you'll be fine.

You'll have a problem fitting a RoW fender to a US car if you plan on keeping the H5s, since the US fenders have tabs on the inside of the buckets where the light assembly attaches. The H4 assemblies are held in place by the trim ring. On H5 cars, the trim ring functions as purely a trim piece.
