If you were to assembly the unit on the ground.

Lay the throw out bearing down on the ground pointing up. shim washer(s), toothed washer (teeth pointing down towards the ground and larger surface end against the pressure plate), pressure plate, thick washer, spring clip.

anyone have any idea why this happened?
galling of the aluminum into the sleeve, the release fork wearing down dramatically, and some journey for good measure.

Somebody needs a new clutch fork. That's about the worst wear I have ever seen on the forks. Was the pedal freeplay adjusted after the last clutch job? It looks like it's been wearing for quite awhile. Was the aluminum guide replaced with the last clutch job? For a cheap part I always replace that guide tube with a clutch.

miles on the car, about 139,000
miles on the clutch and bearings: about 40,000
the guide tube was not replaced with the clutch and bearings, but vertex had said they have never seen one mess up or need a new one.
the aluminum center is all part of the bearing.
the pedal travel was always fine, but after the new clutch i had the vibration, 1.5 years after the install the pedal went to the floor, no leaks apparent but replaced cylinders anyway.
after those, had a weird feeling for the last 3 or 4 years or so with a slight drop in the pedal and not being up all the way, seems the new cylinders never fully corrected the problem.
i have a new fork to put in with all of the rest of the parts.

Sounds and looks like worn out parts to me. Not sure why vertex said the guide tube isn't a wear item, it is.

Using proper lubricant on all contact surfaces help a lot. Driving with foot resting on clutch pedal?
